- 6-12 Students Responsible Use Agreement (Parent and Student Signatures Required)
All entering 6th grade students must have a Tdap vaccine and the date reported to school before the first day of school - September 1. If your student receives any immunizations over the summer, please email student name, date and type of vaccine received to email@example.com
