REGISTER EARLY to address unanticipated problems & avoid delays in getting cleared.
Thank you for helping us to make this process go as smoothly as possible!
Important information about Sports Physicals
At the time of sports registration, athletes must present a CURRENT, ORIGINAL sports physical. Issaquah School District requires ANNUAL physicals. For incoming freshmen, the physical must be dated June 1, 2018 or later. For
ALL athletes, District policy states that sports physicals are good for one calendar year only and cannot expire before the end of post-season events. Print and take the
PHYSICAL FORM to a medical clinic. Physician must complete, SIGN, and DATE the physical form. Before bringing required documents to Liberty for sports registration,
MAKE COPIES OF THE SIGNED PHYSICAL TO KEEP FOR YOUR RECORDS. YOU WILL SUBMIT THE ORIGINAL PHYSICAL TO THE ATHLETIC SECRETARY AT REGISTRATION.
Only ORIGINAL physicals will be accepted - NO photocopies, faxes or scans.
