Eighth Grade Promotion Early Dismissal Permission Form

As a parent or guardian of a student requesting early dismissal following the Eighth Grade Promotion Ceremony on Wednesday, June 26th, I hereby acknowledge that I have reviewed, understand and agree to the following:

  • Student must be an 8th grade participant in the promotion ceremony.
  • Student must arrive on time to first period and attend the ceremony with the 8th grade class.
  • Students may not dismiss themselves. A parent or guardian must escort student after the ceremony.

Permission forms must be completed and turned in to the BLMS Attendance Secretary by Friday, June 21st.  Completed Permission forms can also be submitted electronically at
