NEW STUDENT REGISTRATION FALL - 2018-2019

WELCOME! to BEAVER LAKE MIDDLE SCHOOL!
We are so glad you will have a student attending Beaver Lake Middle School. If you are enrolling a student at BLMS for the 2018-2019 School year please follow these steps.

STEP ONE: Please click on the enrollment link and follow the steps to completing the enrollment forms.

STEP TWO: Please print out and complete the following forms. 

Elective Survey Form With Language Arts, Math and Science Path Forms (These forms are grade level dependent. Please download the appropriate form for your child's grade level.)

You can upload them into your child's registration with the other documents required.

STEP THREE: In addition to the completed online enrollment forms, the following items will need to be scanned and uploaded into the Required Documents section of the student online enrollment. Students will not be enrolled without proof of residency and current immunization records.

 

  • A copy of the student's birth certificate or passport - required to verify student's birthdate and legal name.
  • Immunization record - Students cannot be enrolled without a completed immunization card.
  • Proof of residency is required to verify residency within the Issaquah School District and the Beaver Lake Attendance Boundary. Please include the following:
    • Purchase/lease agreement for the primary residence address. If you are renting or leasing your residence and all utilities are included in the rent, you will need to provide the rental agreement on PROPERTY MANAGEMENT LETTERHEAD ONLY.
    • Utility Bill (The PSE or water/sewer bill must be provided within 30 days.)

 

Once you have submitted the application I will receive an email notification. I will still need you to come into the office with your photo identification and any forms that you did not upload. If you have any questions please call the Beaver Lake Middle School Registrar, at 425-837-4160.
