IMS New Family Information 

Welcome to IMS!

Transitioning to a new school and community can be overwhelming for parents and students alike.  The Issaquah Middle PTSA would love to ease your transition by matching you with an experienced IMS parent to answer your questions.

To be paired with a New Family Ambassador, please share information about your family here: New To IMS Family Information.  What you share will be completely confidential and is used for the purpose of matching you with an experienced IMS parent.

IMS Family Ambassador Volunteer Info

The IM PTSA is starting a great new program called Family Ambassadors Program.  You can make a difference to families who are new to the Issaquah school district and IMS.  

Please consider volunteering for this new and important role!  As an Ambassador, you will serve as a contact person to answer questions and help our new and returning families feel welcome at IMS.  Please tell us about yourself here:  Family Ambassador Volunteer Information so that we may pair you with a new family that has similar interests, background, or grades.  If you have any questions, feel free to contact Wendy Shah at imsnewfamily@gmail.com.

 

What to do if you need an evaluation form from a doctor:

  1. Parents are required to fill out a Release of Information (ROI) for teachers to communicate information about their student to outside agencies. If the student is 13 or older, the student must also sign the ROI.
  2. Contact the office to get the Release of Information form.
  3. Parents need to provide evaluations (from the doctor) with teacher’s name(s).
  4. The office will fax the evaluations to the doctor.
  5. Parents may not pick up the evaluations from the school.
